ISO10993 – HỆ THỐNG ĐÁNH GIÁ SINH HỌC ĐỐI VỚI TRANG THIẾT BỊ Y TẾ
I, Phần giới thiệu:
Giai đoạn đầu, tại
Mỹ FDA đã có những quy định cụ thể đối với các vật dụng liên quan đến thiết bị
y tế,năm 1986 Mỹ, Canada và Anh cùng nhau thiết lập ra " Tri-party
Bio-compatibility Guidance forMedical Devices ", làm cơ sở đánh giá của
các thiết bị y tế khi đánh giá sinh học.
Năm 1992 Tổ chức Tiêu chuẩn Quốc tế công bố ISO 10993-1: 1992 Biological
evaluation of medicaldevices: Part 1: Evaluation and testing。Hiện tại FDA kiểm tra 510(k), PMAđều căn cứ vào「Blue book memorandum G95-1, Use of
International Standard ISO 10993, Biological evaluation of medical device part
1: Evaluation and testing」。
※ Xem xét các đặc tính hóa học của vật liệu
thiết bị y tế, tính chất, mức độ, tần số và thời gian tiếp xúc với cơ thể người.
※ Nhiều thiết bị y tế có kinh nghiệm sử dụng
lâu dài, khẳng định rằng sự an toàn vật liệu vật lý hay hóa học, không nhất thiết
phải thực hiện tất cả các bài kiểm tra theo yêu cầu của hướng dẫn FDA, các nhà
sản xuất chỉ cần hoàn thành tính bản chất là được, nhưng nhân viên kiểm tra FDA
sẽ phán đoán với tính bắt buộc thử nghiệm ngoài.
Do đó, hầu hết các xét nghiệm về khả năng sinh học trên thế giới đều dựa trên “Đánh
giá sinh học thiết bị y tế” ISO 10993.
II, Phạm vi áp dụng:
• Đánh giá tính tương thích sinh học của các thiết bị y tế mà chỉ tiếp xúc
gián tiếp hoặc trực tiếp với cơ thể người.
• Không bao gồm thiết bị y tế không tiếp xúc trực tiếp hoặc gián tiếp với cơ thể
người hoặc các máy móc tạo thành trở ngại có sinh vật gây hại .
• Không áp dụng đối với các vật liệu thiết bị y tế cần có rác dụng của tổ chức
mới có thể phát sinh chức năng đánh giá thiết bị y tế.
III, Việc phân loại thiết bị y tế theo
ISO10993
Dựa vào cách thức tiếp xúc với cơ thể con người được chia thành các loại
sau:
1. Thiết bị y tế không tiếp xúc với cơ thể người: thiết bị y tế
không tiếp xúc trực tiếp hoặc gián tiếp với cơ thể bệnh nhân trong quá trình sử
dụng không có trong phạm vi tiêu chuẩn này.
2. Tiếp xúc bề mặt cơ thể người với thiết bị y tế: bao gồm các phần sau tiếp xúc với thiết
bị y tế:
(1) Da: chỉ toàn bộ bề mặt
cơ thể tiếp xúc với thiết bị y tế, chẳng hạn như các điện cực khác nhau, tay chân giả, chất kết dính, băng nén và thiết bị giám sát.
(2) Niêm mạc: chỉ toàn bộ niêm mạc tiếp xúc với thiết bị y tế, chẳng hạn như kính áp tròng, ống
thông, intravaginal và thiết bị y tế đường ruột (dạ dày, soi đại tràng sigma, nội
soi, nội soi dạ dày), ống nội khí quản, phế quản, phụ nha khoa , thiết bị y tế chỉnh
nha khoa và tử cung.
(3) Chấn thương bề mặt: tổn thương bề mặt cơ thể tiếp xúc với các thiết bị y tế,
ví dụ, viêm loét, ghi băng và các mô hạt chữa lành hoặc thiết bị y tế và , miếng dán kín.
3. Thiết bị y tế kết nối bên ngoài cơ thể:
Bao gồm các thiết bị y tế tiếp xúc với:
(1) Đường máu (không tiếp xúc trực tiếp): thiết bị y tế như tiếp xúc với đường
máu tại một điểm, và là một trong những ống dẫn vào hệ thống mạch máu, chẳng hạn
như một bộ đầu vào chất lỏng (dây truyền) bộ kéo dài, bộ chuyển tiếp và thiết bị
truyền máu (thiết bị).
(2) Mô, xương, răng: với mô, xương hoặc răng tiếp xúc thiết bị y tế, chẳng hạn

(3) Tuần hoàn

4. Thiết bị y tế cấy ghép trong cơ thể:

Phân loại theo thời gian tiếp
xúc của con người:
1. Ngắn hạn (A): thiết bị y tế 1 lần, sử dụng nhiều
lần hoặc tiếp xúc với cơ thể người trong vòng 24 giờ.
2. Dài hạn (B): thiết bị y tế1 lần, sử dụng nhiều lần hoặc tiếp xúc với cơ thể
người trên 24 giờ, nhưng ít hơn 30 ngày.
3. Cố định (C): thiết bị y tế 1 lần, sử dụng nhiều thời gian hoặc tiếp xúc với
cơ thể người trên 30 ngày.
Vật liệu hoặc thiết bị có tiếp xúc với cơ thể người thời gian lâu hơn những loại
trên, phải được kiểm tra nghiêm ngặt hơn. Khi thiết bị y tế nhiều lần tiếp xúc
vào cơ thể người, thời gian tiếp xúc tích lũy phải được tính làm cơ sở để phân
loại.
IV, Nguồn gốc tiêu chuẩn
ISO 10993 "Đánh giá sinh học các thiết bị y tế"
được chia thành 20 chương như sau:

First, the introduction:
Early in the US FDA have been biomedical for medical equipment to develop norms
in 1986, the United States, Canada and the United Kingdom co-developed
"Tri-party Bio-compatibility Guidance for Medical Devices" as a
review of biological evaluation of medical equipment review in accordance with.
1992 International Organization for Standardization ISO 10993-1: 1992 Biological
evaluation of medical devices: Part 1: Evaluation and testing. Currently FDA
review 510 (k), PMA are based on "Blue book memorandum G95-1, Use of

experience confirming the safety of their physical or chemical materials. They
do not necessarily have to perform all the tests required by the FDA
guidelines. Manufacturers simply need to make substantial equivalence
comparisons but FDA reviewers will judge Necessity of additional testing.
Therefore, most biocompatibility tests in the world are based on the ISO 10993
Biological Evaluation of Medical Devices.
Second, the scope of application:
• Evaluate biocompatibility of medical devices that come into direct or direct
contact with the human body only.
• Does not contain medical equipment that is not in direct contact with the
human body or that is caused by mechanical failure.
• Does not apply to the evaluation of medical devices where the material needs
of the medical device and the tissue functions to function.
Third, the classification of medical equipment in ISO10993
According to contact with the human body is divided into the following
categories:
1. Non-contact medical equipment: medical equipment that is not in direct or
indirect contact with the patient's body during use is not covered by this
standard.
2. Body surface access to medical equipment: including the following parts of
the contact with the medical equipment:

various electrodes, prostheses, adhesive plasters, compression bandages and
monitors.
(2) mucosa: contact with the intact mucous membrane medical equipment, such as
contact lenses, urinary catheters, intravaginal and enteral medical devices (stomach,

(3) Injured body surface: Medical equipment in contact with an injured body

3. Externally connected medical devices: Includes medical devices that come

(1) Blood Pathways (Non-Direct Contact): These medical devices come into
contact with a certain point of the blood path and act as a conduit for access
to the vascular system, such as the fluid inlet cuffs, extension cuffs, adapter

(2) Tissue, bones, teeth: Medical devices in contact with tissues, bones or

According to human contact time classification:
1. Short-term (A): Medical equipment single, multiple use of time or contact
with the human body within 24 hours.
2. Long-term (B): medical equipment single, multiple use of time or contact
with the human body more than 24 hours, but less than 30 days.
3. Permanent (C): medical equipment single, multiple use of time or contact
with the human body more than 30 days.
Materials or equipment that come in contact with the human body for more than
one of the above categories may be subject to more stringent tests. When the
medical equipment repeatedly touches the human body, the accumulated contact
time should be calculated as the basis for classification.

一、簡介:
早期在美國FDA中已經有針對醫療器材的生物相容性來制定規範,在1986年美國、加拿大與英國共同制定「Tri-party Bio-compatibility Guidance
forMedical Devices」,作為審查醫療器材生物學評估的審查依據。
1992年國際標準化組織公佈ISO
10993-1:1992Biological evaluation of medicaldevices: Part 1: Evaluation and
testing。目前FDA審查510(k), PMA均根據「Blue book memorandum G95-1, Use of
International Standard ISO 10993, Biological evaluation of medical device part
1: Evaluation and testing」。
※ 考量醫療器材材料的化學特性,與人體接觸的性質、程度、頻率與時間。
※ 許多醫療器材已經有長期使用經驗,證實其物理或化學材料的安全性,不一定要執行所有FDA指引所要求的測試,製造商只要完成實質性等性比較即可,但FDA審查人員會判斷額外測試的必要性。
也因此目前世界上進行生物相容性試驗時大多依據ISO 10993 "醫療器材生物性評估"。
二、適用範圍:
• 僅限間接或直接與人體接觸之醫療器材評估其生物相容性。
• 不包含未與人體間接/直接接觸之醫療器材,或因機械故障造成之生物性危害。
• 不適用於醫療器材之材料需與組織作用才能產生功能之醫療器材評估。
三、醫療器材在 ISO10993 中的分類
依與人體接觸方式分為以下種類:
1.不接觸人體之醫療器材:使用時未與患者身體做直接或間接接觸之醫療器材未涵蓋在本標準範圍內。
2.接觸體表之醫療器材:包括與以下部位接觸的醫療器材:
(1)皮膚:只與完整體表接觸之醫療器材,例如各種電極、體外義肢、固定用膠布、壓迫性的繃帶及監測器。
(2)黏膜:與完整黏膜接觸之醫療器材,例如隱形眼鏡、導尿管、陰道內及腸內醫療器材(胃管、乙狀結腸鏡、結腸鏡、胃鏡)、氣管內管、支氣管鏡、牙科填料、齒列矯正醫療器材及子宮內用醫療器材。
(3)受傷體表:與受傷體表接觸之醫療器材,例如包括用於潰瘍、燒傷及肉芽組織之敷料或癒合之醫療器材及封閉型貼片。
3.體外連通之醫療器材:包括與以下部位接觸的醫療器材:
(1)血液路徑(非直接接觸):此類醫療器材與某一點血液路徑接觸,並當作進入血管系統之一導管,例如液體輸入套(裝罝)、延伸套、轉接套及血液輸注套(裝置)。
(2)組織、骨頭、牙齒:與組織、骨頭或牙齒接觸之醫療器材,例如腹腔鏡、關節鏡、引流系統、牙科用黏粉、牙科充填物及皮膚用釘針。
(3)血液循環:與血液循環接觸之醫療器材,包括血管內導管、暫時性的心律調整器電極、充氧器、充氧器體外導管及配件、透析器、透析管及配件、血液吸附劑及免疫吸附劑。
4.植入體內之醫療器材:包括與以下部位接觸的醫療器材:
(1)組織、骨頭:

3.永久(C):醫療器材單次、多次使用時間或接觸人體時間超過30天。
若材料或器材接觸人體時間可能超過上述一種分類以上,應接受其中較嚴格等級的試驗。當醫療器材多次接觸人體,應計算累計之接觸時間,以做為歸類之依據。
